Most healthcare processes stay opaque until you’re already inside them: you submit a form and wait to find out what happens next. That is not how this works.

Every step below is exactly what you can expect, in order — so you know what you’re signing up for before you request an assessment.

THE PROCESS

STEP 1

Request an Executive Health Assessment

Complete our secure online inquiry form or send us an email — that’s all it takes to begin. From there our team drives the process, and the only other thing we ask before your visit is the intake questionnaire in Step 4.

STEP 2

Personalized Physician Review

Every inquiry is personally reviewed by our physician team — not screened by administrative staff first. Your goals are evaluated to confirm the program is the right fit before anything is scheduled, so you’re never booked into a process that doesn’t suit what you need.

STEP 3

Executive Health Coordinator Onboarding

Once your inquiry is approved, our coordinator reaches out to answer questions, schedule your visit, and send intake forms. This is typically your first direct contact with the practice beyond the initial form.

STEP 4

Complete Your Health Profile

Before your assessment you complete your health history, current medications, lifestyle habits, and sleep questionnaires. This isn’t paperwork for its own sake — it’s what lets your physician spend your assessment time on evaluation and conversation instead of data collection.

STEP 5

You meet your physician for a comprehensive evaluation, typically around two hours, covering everything outlined on the Executive Health Assessment page — followed by a discussion of what your physician is seeing and why.

STEP 6

Personalized Health Strategy

You receive a written physician summary with findings, specific recommendations, and a follow-up plan. It’s yours to keep, reference, and share with other providers if relevant — rather than a verbal summary you have to remember correctly.

STEP 7

Long-Term Physician Partnership

If Executive Membership makes sense based on your goals, this is where that conversation happens — never before, and never as a condition of completing your assessment. Many patients continue with ongoing physician-led care; some do not, and that’s a legitimate outcome too.

Yes, where physician licensure permits. Portions of follow-up care may also be available through telemedicine for established patients.

Yes, when appropriate. Your written health strategy will reflect that coordination rather than operating in isolation from your existing care.

That depends on physician availability and how quickly intake forms are returned. Your coordinator gives you specific timelines once your inquiry has been reviewed.

Your inquiry goes to our physician team for review before anything is scheduled. If the program fits your goals, our Executive Health Coordinator contacts you directly.

Your current medication list, prior test results, and records from other physicians, along with your completed intake questionnaires.

No. Membership is discussed only at Step 7, after your assessment, and only when it fits your goals. Many patients complete an assessment and stop there.

Yes. Many patients bring someone, particularly for the discussion of findings and recommendations.

Our physician team will tell you before anything is scheduled. That is the purpose of the review step, and it means you’re not booked into a process that doesn’t suit what you need.
