No. The Executive Health Assessment is available as a standalone service. Membership is only discussed afterward, and only if it is appropriate for your goals.

Yes. Some patients maintain their existing physician while using Trusted Care for Executive Health, prevention, or a second opinion; others transition fully. Both are common.

Yes, you can travel from another state, subject to physician licensure. Dr. Shamiya is licensed in Texas, Nevada, Arizona, and California.

We aim to respond promptly during normal business hours.

THE EXECUTIVE HEALTH ASSESSMENT

About the assessment

A comprehensive, physician-led evaluation focused on prevention, early risk identification, and a personalized health optimization plan, rather than a routine annual physical.

Approximately two hours, followed by physician review and preparation of your personalized health plan.

Medical history, physical examination, cardiovascular risk assessment, sleep evaluation, body composition analysis, ECG, spirometry, lab review, and a written personalized health plan. Additional testing may be recommended based on clinical findings.

The assessment itself is a self-pay premium physician service. Certain additional tests may be billed through insurance when clinically appropriate.

A written one. You receive a personalized physician summary with findings and specific recommendations that is yours to keep.

EXECUTIVE MEMBERSHIP

About membership

No. The Executive Health Assessment can be completed without enrolling in membership.

It shares some concierge-style access features, but it’s centered on physician-led preventive and personalized medical care, not access alone.

Follow-up frequency is individualized around your health needs and goals. There is no fixed schedule.

Physician services are covered under your membership agreement. Labs, imaging, medications, and specialist services are generally billed separately unless otherwise specified.

Membership terms and renewal policies are outlined in the membership agreement provided before enrollment.

SLEEP MEDICINE

Sleep questions

Because sleep affects nearly every system tied to long-term health and performance, it’s more useful to evaluate it alongside everything else rather than in isolation.

Not necessarily. Sleep testing is recommended only when clinically indicated after physician evaluation.

Yes. Sleep consultations are available independently, though many patients choose to integrate them into their Executive Health Assessment.

Longevity Medicine plans for healthspan across decades. Lifestyle Medicine & Performance Optimization focuses on how you are functioning right now: day-to-day energy, focus, and resilience. The two overlap and inform each other, but they answer different questions.

An annual physical checks what’s already measurable. Preventive Medicine builds a personalized, long-term risk-reduction strategy around your specific history and goals.

For organizations

Yes. Programs are tailored to the size, goals, and needs of each organization. There is no fixed package.

Yes. Individual medical information remains confidential and is managed according to applicable privacy regulations, regardless of who initiated the engagement.

Yes, in multiple states, with services provided according to physician licensure and applicable regulations.

COST & PRICING

What it costs

$2,500. It is a one-time, self-pay comprehensive physician-led evaluation. Additional testing, if clinically recommended, may involve separate charges. See the full cost breakdown on our Cost and Pricing page.

$10,000/year paid annually, or $1,000/month billed monthly. Annual billing costs less over the course of the year. There are no founding-member, introductory, or tiered rates.

Labs, imaging, medications, and specialist visits are generally billed separately unless specifically noted. Physician services are covered under the membership agreement.

Both the Assessment and Membership operate on a non-insurance-based, self-pay model, which is what allows for the extended physician time and personalized approach. Certain individual tests ordered during your assessment may still be billed through insurance when appropriate.

TELEMEDICINE

Virtual care

Yes. Telemedicine is available to established patients, where clinically appropriate and permitted by physician licensure. It is generally used for follow-up care rather than an initial Executive Health Assessment, which is conducted in person.

In many cases, yes, once you are an established patient, subject to physician licensure in your state. Texas, Nevada, Arizona, and California are currently covered.
