Trusted Care Executive Health & Longevity (“Trusted Care Executive Health,” “we,” “us,” or “our”) operates the website at executive.thetrustedcares.com. This Privacy Policy explains what information we collect through this website, why we collect it, who we share it with, and the choices you have.

The Site and the Executive Health program are operated by Mohamed Shamiya PLLC under Trusted Care Clinic and Trusted Care Executive Health & Longevity. Las Vegas Sleep Center serves only as the clinical venue for our Las Vegas, NV location and does not operate this Site or the Executive Health program.

Two different sets of rules apply

Information you give us falls into two categories, and they are governed differently.

1. Website information. Anything you submit through this website before you become a patient — the inquiry form, the corporate partnership form, or an email to our office — along with data collected automatically as you browse. This Privacy Policy governs that information.

2. Protected health information. Your medical record, clinical communications with your physician, intake forms, test results, and billing records created once you are a patient. That information is governed by the Health Insurance Portability and Accountability Act (HIPAA) and by our separate Notice of Privacy Practices, which is provided to you at the start of care and is available on request. Where the two documents differ, the Notice of Privacy Practices controls for your protected health information.

In short: this page is about the website. Your medical record is covered by a separate, stricter document.

Please do not send medical details through this website

Our inquiry form is intended to tell us who you are and what you are looking for — not to transmit your medical history. Do not include diagnoses, medications, test results, or other sensitive clinical details in the inquiry form, the comments field, or an unencrypted email. Once your visit is scheduled, our coordinator will send you secure intake forms designed for that purpose.

This website is also not a channel for urgent care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Do not use this website, the inquiry form, or email to report an emergency or an urgent change in your condition.

Information we collect

Information you provide directly

When you complete our inquiry form, we collect:

When you submit a Corporate Partnership Inquiry, we collect the business contact information and organizational details requested on that form.

We also collect whatever you choose to include when you email executive@thetrustedcares.com or call either office.

Information collected automatically

Like most websites, ours collects certain technical information when you visit:

This is collected through cookies and similar technologies, described below.

Information from third parties

We may receive information from providers who support the website and the practice — our website host, our form and email providers, our analytics provider, and advertising platforms that report on campaign performance. We may also receive information from a person or organization who refers you to us, or from your employer if you participate through a Corporate Executive Health program.

How we use your information

We use website information to:

Cookies and tracking technologies

We use cookies and similar technologies on this website for three purposes:

These tools are configured to measure page visits and campaign performance, not to identify individuals or to draw conclusions about anyone’s health. They do not have access to your medical record, your intake forms, or any clinical information you share with your physician.

Your choices. Most browsers let you block or delete cookies through their settings, though blocking essential cookies may prevent parts of the site from working. You can also manage ad personalization directly with the platforms — through Google’s My Ad Center and through the ad preferences in your Meta account. If you would prefer that we not use your information for targeted advertising, email executive@thetrustedcares.com and we will honor that request.

How we share information

We do not sell your personal information.

We share information only as follows:

Corporate programs. Where an organization sponsors Executive Health services for its executives, the sponsoring organization receives confirmation of participation and invoicing information only. It does not receive clinical findings, test results, or your health plan without your separate written authorization.

Contacting you

By submitting the inquiry form and checking the consent box, you agree that we may contact you about your inquiry using the method you selected. That consent covers the inquiry itself, is not a condition of receiving any service, and you may withdraw it at any time by telling us.

How we protect information

We maintain administrative, technical, and physical safeguards appropriate to the sensitivity of the information we handle, including encryption in transit for form submissions, access controls limiting internal access to those who need it, vendor due diligence, and workforce training on privacy and security.

No method of transmission or storage is completely secure, and we cannot guarantee absolute security. Email in particular is not a secure channel, which is why we ask you not to send clinical details that way.

How long we keep information

We keep information for as long as it is needed for the purposes described in this policy, and for as long as required by law, our professional obligations, and our insurers.

Patient medical records are retained for the periods required by Texas and Nevada law, which are described in our Notice of Privacy Practices.

Your privacy rights

Depending on where you live, you may have some or all of the following rights regarding personal data we hold about you through this website:

Texas residents. The Texas Data Privacy and Security Act provides the rights above. To exercise them, contact us using the details below. We will respond within 45 days, with one 45-day extension where reasonably necessary. If we decline your request, you may appeal by emailing executive@thetrustedcares.com with “Privacy Appeal” in the subject line within 30 days of our decision; we will respond to an appeal within 60 days. If your appeal is denied, you may submit a complaint to the Texas Attorney General at texasattorneygeneral.gov.

Nevada residents. Nevada law gives you the right to direct us not to sell certain personal information we have collected, and Nevada’s consumer health data law provides additional rights concerning health-related data, including rights to consent and deletion. To make either request, contact us using the details below.

California residents. If you are a California resident, you may have rights under the California Consumer Privacy Act. Medical information governed by HIPAA and the California Confidentiality of Medical Information Act is exempt from that law; your rights in that information are described in our Notice of Privacy Practices instead.

Everyone else. Wherever you live, you may ask us to access, correct, or delete website information we hold about you, and we will honor reasonable requests.

How to submit a request. Email executive@thetrustedcares.com with “Privacy Request” in the subject line, or write to us at either office address below. We will need to verify your identity before acting, and we may decline requests we cannot verify. An authorized agent may submit a request on your behalf with written proof of authorization.

Rights in your medical record are separate. Requests to access, amend, or restrict your medical record are handled under HIPAA and state law through our Notice of Privacy Practices, not through the process above.

Children's privacy

This website is intended for adults. We do not knowingly collect personal information from anyone under 18 through this website. If you believe a minor has submitted information to us, contact us and we will delete it.

This website links to third-party sites, including map and directions services and, in some cases, Las Vegas Sleep Center and other affiliated Trusted Care entities. We are not responsible for the privacy practices or content of sites we do not operate. Review their privacy policies separately.

We operate in the United States, and information collected through this website is processed here. If you access this website from outside the United States, you understand that your information will be transferred to and processed in the United States, where privacy laws may differ from those in your location.

Changes to this policy

We may update this Privacy Policy as our practice, our technology, or the law changes. When we do, we will revise the “Last updated” date above. Continuing to use the website after an update means you accept the revised policy.
