These Terms and Conditions govern your use of executive.thetrustedcares.com (the “Site”), operated by Trusted Care Executive Health & Longevity (“Trusted Care Executive Health,” “the practice,” “we,” “us,” or “our”). By using the Site, you agree to these terms. If you do not agree, please do not use the Site.
The Site and the Executive Health program are operated by Mohamed Shamiya PLLC under Trusted Care Clinic and Trusted Care Executive Health & Longevity. Our Richardson, TX office is our primary clinical location; Las Vegas Sleep Center serves only as the clinical venue for our Las Vegas, NV location and does not operate this Site or the Executive Health program.
These terms govern the website. They do not govern your medical care. Care is governed by the consents, financial agreements, and — if you enroll — the Executive Membership Agreement you sign with the practice directly.
Everything published on this Site — service descriptions, the pages on sleep, preventive, functional, longevity, and lifestyle medicine, our FAQs, and any articles or downloads — is general information about how our practice works. It is not medical advice, diagnosis, or treatment, and it is not a substitute for a consultation with a qualified physician who knows your history.
Do not use this Site to diagnose yourself, to decide whether to start, stop, or change a medication, or to delay seeking care. Always consult your physician or another qualified health professional about your specific situation.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
Our inquiry form, email address, and office phone lines are not monitored continuously and are not an emergency service. Do not use them to report an emergency, a suicidal or self-harm crisis, or a sudden change in your condition.
Reading this Site, submitting the inquiry form, emailing us, or speaking with our coordinator does not create a physician-patient relationship.
A physician-patient relationship is established only when a Trusted Care Executive Health physician accepts you as a patient, you have completed the required intake and consent documents, and care has begun. Until then, no physician here has any obligation to diagnose, treat, or advise you.
We review every inquiry personally and we may decline one. We are not obligated to accept any person as a patient, and we may decline an inquiry for any lawful reason, including clinical fit, physician availability, or licensure limitations.
Inquiry submissions and email are not secure clinical channels. Please limit what you send to the information the form asks for, and do not include diagnoses, medications, test results, or other sensitive medical details. Once your visit is scheduled, we will send you secure intake forms designed for clinical information.
You are responsible for the accuracy of what you submit. Submitting another person’s information without their permission is not permitted.

You must be 18 or older to use this Site and to submit an inquiry. By using the Site, you represent that you are at least 18 and legally able to enter into these terms.
We publish our pricing openly, and we intend the published figures to be accurate:
Both are non-insurance-based, self-pay services. We do not bill insurance for the Assessment fee or the Membership fee.
Prices published on this Site are current as of the last update and may change. The price that applies to you is the price confirmed in writing by our team before your visit is scheduled or your membership begins. We will not change a price after you have been quoted and confirmed it for a service you have already paid for.
What the fees do not include. Neither fee covers laboratory testing, imaging, medications, or specialist visits unless specifically stated in writing. Additional clinically recommended testing performed during or after your assessment is billed separately. Your physician will discuss any recommended testing, and its cost, before ordering it.
Insurance. Certain individual tests or specialist referrals may be billed through insurance where appropriate. Whether your insurer covers any of it is between you and your insurer. We make no representation that any service will be reimbursed.
HSA/FSA. Eligibility depends on your plan administrator and how the fee is categorized. Confirm with your plan before enrolling. We can provide documentation of services on request, but we cannot guarantee your administrator’s determination.
Medication samples, where provided, are furnished only when clinically appropriate and available, at no charge, and are not a substitute for an ongoing prescription supply. No sample is guaranteed.
Membership is a separate written agreement between you and the practice. The Executive Membership Agreement, not this page, governs your membership — including the term, billing schedule, renewal, cancellation, refunds, and exactly what physician services are covered.
You will receive that agreement before enrolling, and we recommend reading it in full. Nothing on this Site modifies it. Where this Site and the Membership Agreement conflict, the Membership Agreement controls.
Membership is individual. Each person enrolls separately. Membership is not insurance, is not a health plan, and is not a substitute for health insurance coverage.
Corporate programs are arranged under a separate written agreement with the sponsoring organization. Terms, pricing, and scope for corporate engagements are set in that agreement. Participation by an individual executive remains subject to the same clinical judgment, consent, and confidentiality protections as any other patient — a sponsoring employer does not receive clinical findings without the individual’s separate written authorization.
Telemedicine is available to established patients only, where clinically appropriate and where physician licensure permits. It is not part of the standalone Executive Health Assessment, which is conducted in person.
Our physicians may provide care only in states where they hold an active medical license. Dr. Shamiya is licensed in Texas, Nevada, Arizona, and California. Licensure status can change; your coordinator will confirm what is available for your location when you schedule. Nothing on this Site is an offer to practice medicine in any jurisdiction where our physicians are not licensed.
Telemedicine has inherent limitations, including the inability to perform a physical examination. Your physician may determine that an in-person visit is necessary.
Appointments are confirmed by our coordinator. Any applicable cancellation or rescheduling terms will be disclosed before the appointment is finalized.

All content on this Site — text, page structure, photography, graphics, logos, and design — is owned by Trusted Care Executive Health & Longevity or its licensors and is protected by copyright, trademark, and other laws.
“Trusted Care Executive Health & Longevity,” “Trusted Care,” and Get Your Sleep Right™ are marks of the practice or its affiliates and may not be used without written permission.
You may view, download, and print pages from this Site for your own personal, non-commercial reference. Any other use — reproduction, republication, distribution, modification, or derivative works — requires our prior written consent.
Your written health plan. The personalized health strategy prepared for you following your Executive Health Assessment is yours to keep and to share with any other physician. That permission is specific to your own plan and does not extend to other Site or practice materials.
Physician credentials, board certifications, licensure, and affiliations described on this Site are accurate as of the last update. Credentials and licensure can change. Nothing on this Site should be read as a representation that a specific physician will be available to you, or that any particular physician will provide your care. Your coordinator confirms your physician when your visit is scheduled.
Any patient reviews, testimonials, or review counts referenced on this Site reflect the experience of individual people. They are not a promise, guarantee, or prediction of any result for you. Health outcomes depend on factors outside our control, including your medical history, genetics, adherence, and circumstances.
We make no guarantee of any specific clinical outcome, diagnosis, improvement, prevention of disease, or extension of lifespan or healthspan from any service described on this Site.
Where functional medicine or longevity medicine principles are incorporated into care, they are used only where our physicians judge them clinically appropriate, and always within evidence-based practice. They are offered as part of conventional physician-led care, not as an alternative to it. Some approaches in these fields are less established than others; your physician will tell you where the evidence is strong and where it is not.
The Site links to third-party websites and services, including mapping and directions providers and affiliated entities such as Las Vegas Sleep Center. We do not control those sites, do not endorse their content, and are not responsible for them. Your use of a third-party site is governed by that site’s own terms.
We do not guarantee that the Site will be available without interruption, error-free, or free of harmful components. We may modify, suspend, or discontinue any part of the Site at any time without notice.
To the fullest extent permitted by law, the Site and its content are provided “as is” and “as available,” without warranties of any kind, express or implied, including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement.
This disclaimer applies to the website and its content only. It does not apply to, limit, or disclaim the professional duty of care owed to you by your physician once a physician-patient relationship exists.
To the fullest extent permitted by law, Trusted Care Executive Health & Longevity and its physicians, employees, and affiliates will not be liable for any indirect, incidental, consequential, special, or punitive damages arising from your use of, or inability to use, this Site or any content on it.
Important carve-out. This section limits liability arising from the website. It does not limit, waive, or disclaim any liability arising from the professional medical services we provide to patients, and nothing in these Terms should be read as an attempt to do so. Claims relating to medical care are governed by applicable Texas or Nevada law, not by this section.
Some jurisdictions do not allow certain limitations of liability. Where that is the case, the limitations above apply only to the extent permitted.
You agree to indemnify and hold harmless Trusted Care Executive Health & Longevity and its physicians, employees, and affiliates from claims, losses, and expenses (including reasonable attorneys’ fees) arising from your misuse of the Site, your violation of these terms, or your infringement of another party’s rights. This does not apply to claims relating to the professional medical services we provide.
We want this Site to be usable by everyone. If you encounter an accessibility barrier, email executive@thetrustedcares.com or call either office and tell us what you ran into. We will work with you to provide the information you need in an accessible format.
Your use of the Site is also governed by our Privacy Policy. If you become a patient, your protected health information is governed by our separate Notice of Privacy Practices under HIPAA.
We may revise these terms at any time. Revisions take effect when posted, and we will update the “Last updated” date. Continuing to use the Site after a revision means you accept it. If a change materially affects your rights, we will make that clear on this page.
If any provision of these terms is found unenforceable, that provision will be limited or removed to the minimum extent necessary and the rest will remain in effect. Our failure to enforce a provision is not a waiver of it.
These terms, together with our Privacy Policy, are the entire agreement between you and us regarding the Site. They do not replace or modify any signed agreement between you and the practice, including consent forms, financial agreements, or the Executive Membership Agreement — those control over this page on their subject matter.
